Assuming you’re applying using your **Italian passports**:

1. **Other-country passports or identity cards:** Answer **Yes** and enter your Israeli passport details, using the **passport number**, not your Israeli national ID number. The question specifies identity cards **“for travel”**; an ordinary Israeli identity card is not an international travel document, so my reading is that you do not need to add it in this section. This distinction is also explained in [this answer about the same question](https://travel.stackexchange.com/a/172022).

A separate field labelled **“National Identification Number”** is a different question—do not confuse it with the document-number field for another passport or travel identity card.

2. **Your father’s employment:** Answer **Yes** and report his **current self-employment**. He can enter **SELF-EMPLOYED** as the employer name and provide his business address, using his home address if that is where he works. He does not need to substitute his employer from 15 years ago. The [CBP ESTA online help](https://www.reginfo.gov/public/do/DownloadDocument?objectID=50695601) explicitly allows “self-employed” as an employment description.
